1. From your dashboard, click on "Donation Campaigns"

2. The type of donation form will be listed under "Type"

3. Select the donation campaign by clicking "Manage"

4. Click "Settings"

5. Click "Custom Questions" on the top menu

6. Click "Create a form"

7. You will now see a list of different types of Custom Questions! You can choose as many or as few as you like. You can also make them required or not required.

8. You can use these steps for any type of donation form!
